Ready to apply? The application is below. It is several pages, and you can finish it at your own pace before your program tour.
How the application works
- Complete the Demographic Information Request below and pay the $75 application fee.
- Our Director of Admissions calls you within 24 business hours to schedule your program tour. Tours can be virtual, on Microsoft Teams.
- You receive an automated email explaining where to send previous IEPs and any educational, psychological or psychiatric evaluations for our clinical team to review. You can also email them to Admissions@NDFYA.com or fax (954) 571-5265 before your tour.
- Within 24 hours you also receive a link from Ping MD by email to complete the rest of the application.
What happens at the first meeting
Students and parents or guardians usually attend the intake appointment together, though this is not required. We talk through our service recommendations and the costs, and you tour the apartment complex and the residences where our students live.
The application can be completed at your leisure before your tour. This information helps our staff customize the individual service plan (ISP). All information in the electronic database is kept confidential.
